W-9
Purpose: A W-9 is an IRS document certifying the TIN (tax identification number) provided for payment of income, real estate transactions, mortgage interest, acquisition or abandonment of secured property, and cancellation of debt or contributions to an IRA.
Entities paying Kent State will require a W-9 from Kent State certifying that the identification number given is correct and /or to document that Kent State is tax exempt.
To request a W-9:
- Contact either Vicki Ladd at vladd@kent.edu or Paula DiVencenzo at pdivence@kent.edu.
- Indicate name and address of entity requesting the W-9
- Include an e-mail address or fax number to send W-9 once completed
If your department receives a vendor's completed W-9 , please forward to Accounts Payable. Accounts Payable is required to obtain updated W-9's from vendors every 2 years. Vendors who do not return the W-9 may either have their payments suspended or may be subject to a tax withholding that will reduce their payment.
