Consult  with a faculty research mentor and complete this form for permission to enroll for this course. The course is for students seeking a specific and extensive research experience.

Approval for "Internship in Biological Sciences"  (BSCI 40192)  [pdf]

Consult  with an internship mentor and the department Undergraduate Coordinator and complete this form for permission to enroll for this course. The course is for students seeking a specific and extensive research experience with a company or another academic institution.

Biology majors interested in pursuing an off campus internship experience with the intent of receiving course credit must follow the directions of this form and receive approval from the BSCI Undergraduate Coordinator.

   Permission for Time Conflict Override  [pdf]

In order to receive a time conflict override, use this form when two courses have an overlapping time conflict. The student must collect signatures from both course instructors that are in question and turn the form into the BSCI main office, CHH 256.

   Permission to Enroll  [pdf]

In order to receive a prerequisite override, use this form when the student does not meet the course prerequisites listed in the university catalog. Student will need to request permission from course instructor, complete the form and turn into the BSCI main office, CHH 256.

   Petition for Exception to Undergraduate Program Requirements  [pdf]

Use this form when a student requests a course substitution in the degree program or requests transfer coursework credit. The form must be completed and signed by both the student’s BSCI faculty advisor and by the BSCI Undergraduate Coordinator before being sent to the College of Arts and Sciences.
