Application Packet:  (PDF format) Use this option if you prefer not to apply online. Complete in all the forms, print and mail.

Checklist for Applications: This list will help you make sure that you have all of the forms that you need and get them submitted to the proper addresses.

The following forms can be filled out and/or printed separately as needed:

Application: 3 copies of the completed application are required

Letter of Recommendation: 3 letters of recommendation from current or previous professors are required

Statement of Goals: Describe why you are seeking a graduate degree. 

Assistantship Application: (Optional) Please fill out only if you are interested in a graduate assistantship with our school.

• To start your assistantship in Fall Semester, you should have your completed application in by February 1st.
• To start your assistantship in Spring Semester, you should have your application in by November 1st.

Ethnic Data: This form is requested by Kent State to assist them in its reporting requirements.
