Undergraduate Forms

Please completely fill out and print the following forms before turning them in to the main Communication Studies office in room 135 Taylor Hall for processing.

Authorization or Permission to Enroll	This form is to be used to request permission to enroll in a class if it is full or you do not meet the minimum requirements to register for the class. An authorization will be issued to you to override restrictions on the class but will not allow an overload. A permit must be issued to overload a class. Only the instructor of the course and section that you are interested in can approve this.
Application for COMM 46091 Senior Seminar	Once you are a senior and are ready to take COMM 46091 you will need to fill out the top portion of this form before you can register for this course. You need to have completed 33 credit hours of Communication courses prior to the semester you want to enroll in Senior Seminar. Generally the course should be taken during your last semester. Please be sure to list in order of preference ALL sections that you can take. We will try to accommodate everyone's first choice but that isn't always possible. If you cannot take a sections, please include the specific reason.
COMM 45092 Internship	Along with this application, you must include a letter from the company that you will be doing your internship with that includes what your duties will be and the hours that you will be working. This should be on their company letterhead or another formal document produced by the company and signed by your proposed supervisor. You will also need to get their signature on page two of this application (project director).
COMM 46092 Practicum in Applied Communication	Along with this application, you must include a letter from the company that you will be doing your practicum with that includes what your duties will be and the hours that you will be working. This should be on their company letterhead or another formal document produced by the company and signed by your proposed supervisor. You will also need to get their signature on page two of this application (project director). Please be sure to print the additional COMM 46092 documents below.
COMM 45196 Individual Investigation	This application needs to be filled out to request registration in this course. You must have your project described on the application and the faculty member's signature on page 2 of the form (project director) showing their willingness to work with you on your desired project.
