Authorization or Permission to Enroll
This form is to be used to request permission to enroll in a class if it is full or you do not meet the requirements to register for the class.
An authorization will be issued to you to override restrictions on the class but will not allow an overload. You may not use this form if you are currently enrolled in Foundations of Communication (COMM20000).
A permit will be issued to allow you to overload into a closed class.

Application for Senior Seminar COMM 46091
You need to fill out the top portion of this form before registering for this course. You need to have 33 credit hours of Communication courses completed prior to the semester you want to enroll in Senior Seminar. Generally this course should be taken your last semester.

Please be sure to list in order of preference ALL sections you can take. We try to accommodate everyone's first choice but that isn't always possible. If you cannot take a section, please include the reason.

Internship and Individual Study Application COMM 45092, 46196 and 46092
For COMM 45092 and 46092 Please also include a letter from the company that you will be doing your internship with stating what your duties will be and the hours you will be working. This should be on their company letterhead or another formal document produced by the company. You will also need to get your proposed supervisory's signature on page two where it says Project Director.

For COMM 46092 please see addition forms you need to print out below.

For COMM 46196 you need to get prior approval from a faculty member that will agree to supervise you through your individual study.
