When your child is no longer your dependent and/or is leaving your household, these are the steps to consider to make the necessary changes to your benefits.
- Complete a Change of Employee Personal Information form to remove a child from medical/dental and vision plans and to end tuition waiver eligibility.
- Submit all completed forms within 31 days.
- Information about how to continue medical/dental and vision coverage for the child under COBRA will automatically be mailed. To obtain rate information in advance you may contact the Benefits Office 330-672-3107.
- Contact Human Resource Services at 330-672-2901 or the Payroll Department at 330-672-2637 to make appropriate changes to your tax forms for federal, state or local taxes.
