Requesting Leave Donation:
Employees wishing to apply for Leave Donation must complete the Leave Donation–Employee Leave Request, in addition to the Leave Request Form. The employee’s physician must complete the Physician Certification prior to any employee being approved for Leave Donation. Completed forms are to be returned to the University Benefits Office at Wright Hall, 1st Floor.
Employees wishing to donate accrued time must complete the Leave Donation–Donor Authorization Form. The completed form should be returned to the Benefits Office at the Terrace Hall Annex. Any employee requesting or donating leave should read the Leave Donation Guidelines and Procedures and policy 3342-6-11.3 prior to requesting or donating leave.
Policy:
Policy 3342-6-11.4 establishes a leave donation program which allows employees to voluntarily donate paid leave to employees who are on unpaid temporary disability leave (under Policy 3342-6-11.3).
An employee may receive donated leave up to the number of hours the employee is normally scheduled to work each pay period, if the employee who is to receive the donated leave:
Has a warranted medical condition, and Has a zero balance of sick leave and vacation leave and is expected to have a period of unpaid leave of at least two weeks, and Has not applied for nor is receiving worker’s compensation benefits for lost wages.Employees may donate sick or vacation leave if the donating employee:
Voluntarily elects to donate leave and does so with the understanding that donated leave will not be returned, and Donates a minimum of eight hours of sick and/or vacation leave initially with subsequent donations being made in increments of four hours, and Retains a sick leave balance of at least 240 hours.
