Vision Insurance
A separate vision plan, administered through Aetna, Inc., covers employees and their dependents who are enrolled in any health care plan option. Benefits are provided for vision examinations, frames and lenses, and contact lenses. Maximum benefit levels are renewed on a rolling 24-month period.
There are no ID cards issued by Aetna for the vision plan. To submit claims to Aetna, simply print an Aetna claim form and have your vision provider complete the “physician or supplier” portion. Submit your claim to the address on the form and reimbursement will be issued to the employee by Aetna.
