SEVIS (I-20, DS-2019) Information
IMPORTANT! READ AND FOLLOW THIS PROCESS AFTER RECEIVING FORM I-20 OR DS-2019.
Step 1: Verify your personal information on your Form I-20 (F-1) or DS-2019 (J-1). Ensure that your name and birth date match your passport. If you find mistakes notify our office immediately.
Step 3: Prior to applying for your F-1 or J-1 student visa you must pay a $200 SEVIS fee to the U.S. Department of Homeland Security (DHS). You can pay the SEVIS fee two ways: (1) online by credit card or (2) mailed by check or money order. You will be able to print a receipt electronically if you use payment option #1. You will receive a receipt by return mail if you use payment option #2.
There are special instructions for students from Bermuda, Canada, China and India.
OPTION #1: ONLINE PAYMENT BY CREDIT CARD
For electronic submission of Form I-901* and payment of the SEVIS fee using a credit card, go to www.fmjfee.com. Follow the on-line instructions. Print out the payment screen to verify your payment. Take the payment verification printout with you to your visa interview. The fee must be paid at least three business days prior to the scheduled date of your interview in order for the payment information to show up on the SEVIS system.
We strongly recommend that you use this option if possible.
OPTION #2: MAIL PAYMENT BY CHECK OR MONEY ORDER
When mailing payment, the check or money order, payable to “I-901 Student/Exchange Visitor Processing Fee,” must be made in U.S. dollars and drawn on a bank located in the United States.
There are two options:
a. Internet-generated coupon*. Students can go to the fee payment website www.fmjfee.com, enter basic information, print out a coupon, and then mail a check or money order with the coupon to a lock-box address in Missouri (below).
b. Paper option. Students can download or otherwise obtain Form I-901*, fill it in, and mail it, with a check or money order, to the specified address in Missouri (below).
Once the information and fee are processed, SEVIS is updated with the fee payment information and a receipt notice is issued via postal mail. The mailing addresses for mail payment are:P.O. Box Address:

DHS will accept fee payment from a third party individual or institution (other than the student), either in the United States or abroad, using either option #1 or option #2 above.*COMPLETING FORM I-901:
a. You will need information from your Form I-20 or DS-2019 in order to complete the I-901.
b. You will need Kent State University’s School Code. Our School Code is CLE214F00387000 for F-1 students (I-20’s) and P-1-02089 for J-1 students (DS-2019).
STUDENTS FROM CANADA OR BERMUDA
Students from Canada and Bermuda are exempt from typical visa requirements. Instead, those students should provide SEVIS fee payment verification along with Form I-20 or DS-2019 to a Customs and Border Patrol Officer at the port of entry when applying for entry into the United States as an F-1 or J-1 student.
If you have obtained an I-20 or DS-2019 for a spouse or children, they do not have to pay a SEVIS fee in order to apply for an F-2 or J-2 visa.
IF ADMITTED TO MORE THAN ONE SCHOOL
If you have been accepted to more than one institution and paid the fee using the SEVIS ID number of another institution but you wish to attend Kent State, you will not have to pay the fee again. Bring the I-20s of both the schools to the consulate or port-of-entry and tell the officer you wish to attend Kent State University.

Step 4: Schedule the visa interview. The documents you need for the interview are: valid passport, Form I-20 or DS-2019, your I-901 receipt and financial documents showing sufficient funds to study abroad. The officer will review the documents and return all documents to you. Follow these tips when applying for a non-immigrant visa.
Step 5: Upon visa approval, begin making travel arrangements. Schedule an arrival date that allows you to attend the orientation session. Complete the Arrival Form online and indicate your exact arrival date, flight/train/bus number, and arrival time by August 7, 2012.
Step 6: During flight, NEATLY complete Form I-94 (white card – Arrival/Departure Record), using consistent spelling of your name to match your passport and Form I-20 or DS-2019. This information is scanned electronically and will be used to process future application for Social Security Number as well as arrival information. If information is unclear, it may result in an unnecessary delay for you.
Step 8: Upon arrival at Kent State University report to the International Student and Scholar Services Office, 106 Van Campen Hall, 625 Loop Road. You must present your passport, Form I-94 (white card – Arrival/Departure Record) and Form I-20 or DS-2019. The same documentation is required for any accompanying dependents. Failure to report to the school listed at Number 2 on your Form I-20 will be reported to the United States Citizenship and Immigration Services (USCIS) as a violation of your immigration status.
