Fine Appeal Form
To appeal a library fine, you may fill out the form below and submit it for review. Circulation Services staff will contact you within three business days with the results of the appeal. Please refer to our Policy on Waiving of Fines, Fees, and Other Charges.
State below the reason(s) for your appeal. If your appeal is approved, supporting documentation will be required before completion.
To pay your fine, you can stop at the Circulation Desk at the main library, or pay online using a credit card.
