Per the Department of Education, a student that needs to complete this form, MUST appear in person at the Kent State University Financial, Billing and Enrollment Center or a Regional Campus student services area to verify their identity by presenting an unexpired valid government-issued photo identification (ID) such as, but not limited to, a driver's license, other state-issued ID, or passport. The student must also sign the form in the presence of the institutional official.
