University Administrative Policy requires all university employees who operate motor vehicles owned or leased by the university to have their operator license reviewed on an annual basis.
University Administrative Policy #3342-5-12.5 states:
Please forward this information using the Employee Motor Vehicle Operator License Validation form no later than September 30th each year so that a prompt check can be administered for you and your employee(s). Also forward a validation form with new employee information for an initial check when an employee begins driving for your department regardless of the time of year. The charge for each motor vehicle operator license check will be $8.50 for Ohio and $18.00 for each out-of-state license check. Therefore, please supply the account name and number to IDC for these license checks along with the name and phone number of the contact person in the event that a problem is discovered.
