4. If you are using children 12 and under you must use an assent script (Link)
5. If you are recruiting research participants by telephone, you must use a telephone script 
6. If you are using deception you must complete this questionnaire (Link)
7. If you are using nutritional supplements you must complete this questionnaire (Link)
8. If you are drawing blood you must provide proof of certification
