When does my project expire, do I have to submit an annual review?

The IACUC office may send an annual review/new protocol due reminder notice to you by email as a courtesy. However, please note that it is the responsibility of the principal investigator to be aware of the study expiration date and submit the required materials in a timely manner. Operating outside of approval dates is considered noncompliant. Please remember: After three years (the original protocol approval, followed by two (2) annual reviews), a new protocol submission is required. Please submit annual review forms at least 2 weeks prior to the expiration date. For projects continuing beyond three years please submit the necessary forms no later than 6 weeks prior to their expiration. Please remember to observe meeting dates should your project require full board review.

How do I modify my study?

You must complete, submit, and receive the approval of a modification request form. This form can be found at IACUC Forms.
