When ordering an exact reprint of a publication, be sure to provide the job number of the most recent version. Job numbers are often located in the lower right-hand corner of the back cover or last inside page of publications (for example, 01-4395 or 02-8766). To place your order, please

- Legibly print the required information; and
- Fax the completed form to University Communications and Marketing at 330-672-2047.
- Use a separate sheet for each order.
