Students with current health insurance coverage who visit the Health Center without their insurance information will have two (2) business days to provide the appropriate information.  If insurance information is not provided within two (2) business days, fees will be assessed at the self-pay rate to the student's Bursar account.

Students may provide the information by returning to the Health Center Insurance & Billing Desk, by calling us at 330-672-8251, or by faxing a copy of both sides of the insurance card to 330-672-2272.

Attn: Insurance & Billing. Please also include patient's name and sender's name and phone number in the fax.

Required insurance information:

  • Insurance company name
  • Claims submission address
  • ID number
  • Group number
  • Customer service phone number
  • Primary insurance holder's name and date of birth
