Students with current health insurance coverage who visit the Health Center without their insurance information will have two (2) business days to provide the appropriate information. If insurance information is not provided within two (2) business days, fees will be assessed at the self-pay rate to the student's Bursar account.
Students may provide the information by returning to the Health Center Check-Out and Billing Desk, by calling us at 330-672-8251, by faxing a copy of both sides of the insurance card to 330-672-2272 Attn: Central Billing Office, or by emailing both sides of the insurance card to firstname.lastname@example.org. Please include the patient's name, KSU student ID number, phone number, and the sender's name if different than the patient in any fax or email.
