“Know Your Numbers” – Fall On-site Health Screenings and Flu Clinics

As part of the Wellness Your Way incentive program, full-time, benefits-eligible employees are invited to attend an on-site health screening which will count toward their 2019 Tier One Wellness Your Way requirements.

In addition, we have arranged flu vaccine clinics to align with all upcoming health screening dates through December 2018 for all campuses.

Completing Your Tier One Activities

If your health screening is completed on-site with Be Well Solutions, Kent State University's wellness partner, it will automatically be loaded into your personal portal. If your health screening is completed in your physician’s office, please log in to the Be Well portal, under “My Wellness,” select “Wellness Reward Program,” print the “TIER 1 – PHYSICIAN SCREENING FORM” and take it with you to your appointment. Your completed form should then be submitted to Be Well Solutions using the address on the form.

The following on-site dates are now available to schedule your “Know Your Numbers” health screening and/or flu vaccine. Follow the “How to Schedule” instructions below as registration is required.
