Don’t Miss Out On Your Opportunity to Earn Your $300 Wellness Reward

Tier One Requirements due by April 30, 2018

Kent State University’s Employee Wellness Program has awarded more than $100,000 to faculty and staff to date for 2018. As part of the Wellness Your Way employee incentive program, all full-time, benefits-eligible employees are invited to attend an onsite health screening (you do not have to be covered by the university-sponsored health plan to take advantage of this on-site service).

Screenings conducted between September 2017 and April 30, 2018, will count toward the employee Tier One requirement for 2018. Once all requirements (health screening, health risk assessment, Wellness Waiver form) are complete for Tier One, eligible employees will receive their Tier One $150 wellness reward. The deadline for completing Tier One is April 30, 2018.

If you have not already scheduled an onsite health screening, there are limited openings available for a final screening date on April 25.

Registration is required for the health screening, and you may do so at portal.bewelldata.com. Once logged in, go to “My Appointments,” “Worksite Screening” then select the date and time you prefer. If you have never accessed the Be Well portal, you will need to follow the first-time login instructions.

You May Also Complete the Necessary Screening with Your Personal Physician

If you already completed a health screening on or after Sept. 1, 2017, you do not have to complete another one for 2018. Also, if you already had or prefer to have your health screening done in your physician's office, simply print the Physician Screening Form from the Be Well website at portal.bewelldata.com and have your physician complete the form and send the results directly to Be Well at the address shown on the form by April 30. Note that screenings would have had to be completed between Sept. 1, 2017, and April 30, 2018, to count toward Tier One for 2018.
