Completing Patient Portal Electronic Forms
- Visit https://kent.medicatconnect.com
- Select "Forms" - either from the top list of tabs or in the center of the page under the "To Do List"
- Browse under the "UHS Forms" section
- Click the link for the form(s) that you have been asked to fill out (e.g. "Consent for Treatment") to be taken to web-based version of the form
- Complete the appropriate requested information, remembering to electronically sign (type your name) and date the forms as appropriate and then click the "Submit" button at the bottom of the page
- You should now be directed back to the "Forms" page where you can click another form and complete it if needed
