Required Camp Forms
The safety of your camper is our #1 priority! We use these forms in order to keep your camper safe, and make our staff aware of any allergies, medications, or special accommodations.
Forms are due by 5 p.m. on the Thursday prior to the week which your camper is attending PEAK. (i.e. The forms for week 3 are due by Thursday of week 2)
The Hold Harmless Waiver, and Camper Health History forms are required to be filled out for each camper who attends PEAK, but they only need to be completed once for the entire summer! Both should be accessible during the registration process, or you can find them listed below.
