Guidelines and Forms for Requesting to Return to Research/Creative Activities
Updated procedures for Returning to Research
The Research and Creative Activities Reopening Committee has provided procedures for a phased return to research and creative activities (PDF). These procedures were developed with the overarching aim of keeping the entire Kent State University family safe, while increasing research/creative activity in a phased approach as safety becomes easier to maintain.
Faculty interested in returning to their labs/studios at this time should complete and submit the Request to Return to On-Campus or Off-Campus Research Qualtrics survey. Individuals who oversee space for your unit (e.g., chairs, deans, directors) will review applications. If you need to order cloth or paper masks, gloves, or spray sanitizer / disinfectant for your lab/studio you can follow these instructions and submit a work order. To order face shields, gowns, or alcohol-free wipes follow these instructions.
In addition to the Request to Return to Research forms, faculty/research group leaders and trainees/staff are also required to complete the relevant Return to Research Terms and Conditions for Research Group Leaders or the Return to Research Terms and Conditions for Trainees/Staff forms.

If you are interested in returning to in-person human subject research, but are unable to adhere fully to the requirements of the current Return to Research risk level, you can apply to the Risk Mitigation Subcommittee for an exception. Risk Mitigation Approval Request Forms should be submitted to Doug Delahanty at email@example.com. Please put RISK MITIGATION APPROVAL REQUEST in the subject line of your email.
If you would like to add personnel to an approved protocol and are not changing the protocol in any other way, you can have these requests approved directly by your space approver. You must still ensure that all personnel are trained on the research protocol and safety requirements and that you have collected their Terms and Conditions form. Below please find suggested email language for submitting a request to your space approver.
Dear [whoever is your approver]:
I would like to add the following persons to my currently approved Return to Research protocol. There are no changes to the research; I am just adding personnel. All personnel have been trained on the research protocol and our safety requirements, and I have collected each one’s Terms and Conditions attestation. Here are the additional personnel:
John Smith, undergraduate
Jane Smith, graduate student
Please let me know if these additional personnel are approved. Thank you
