COVID-19 Vaccination Exemption

On Aug. 27, 2021, Kent State issued a COVID-19 vaccine requirement effective Dec. 20, 2021. Exemptions to this requirement will be granted for medical, religious or personal conscience reasons.

Anyone seeking an exemption must submit the appropriate request form (see below) and accompanying documentation. Submitting the request form and appropriate documentation does not grant you an automatic exemption. Exemption requests will be individually reviewed for completion, accuracy of information and direct relevance to the COVID-19 vaccination requirement. 

Please do not assume that because you have requested an exemption that it is or will be approved. Submitting your request starts the review process. You will be notified, in writing, whether your request is approved or denied. Denials cannot be appealed, but you may submit a new exemption request if additional supporting documentation becomes available.

The review process is expected to take up to four weeks, after the submission of the appropriate request form and documentation.

How do I access the COVID-19 exemption request forms? 

Selecting the Correct Exemption Request Form

The first thing to consider is which form do you need? Use the following information to decide which request form to utilize. If you use the incorrect request form your request will be automatically denied. 

Anyone submitting an exemption request must use the appropriate form for their given population prior to complete review of the request. 

Students

For the exemption process, students are defined as people enrolled in classes at Kent State University who are 18 years or older and who are not also classified or unclassified Kent State University employees. 

Minor Students

For the exemption process, minor students are defined as people enrolled in classes at Kent State University who are younger than 18 years of age and who are not also classified or unclassified Kent State University employees. These minor students must have their parent or legal guardian contribute to the exemption request as appropriate on a given request form. 

Employees

For the exemption process, employees are defined as people who hold classified or unclassified employment positions at Kent State University. For the purposes of this exemption request process, people holding classified or unclassified positions that are also enrolled in classes are to be considered as employees and not students. 

Adult Student with a Legal Guardian

For the exemption process, adult students with a legal guardian are students who are 18 years old or older who have been assigned a legal guardian by the court to act on their behalf. For these students, their legal guardian should complete the Adult Student with a Legal Guardian form that meets their exemption request need.

Printed copies 

As part of using a printed copy you must read the CDC COVID-19 Vaccine Information prior to filling out the request form. You must attest to having read this information as part of the request, so please make sure to open the link above and read the page prior to completing the request form. 

On the Kent campus 

Printed copies of each type of COVID-19 vaccination requirement exemption form are available in the front entrance of the DeWeese Health Center ( 1500 Eastway Dr Kent, OH 44242 ) during the Health Center’s hours of operation, which can be found on the University Health Services web page

On a regional campus  

Printed copies of each request form are available at the Student Services reception desk at each regional campus. 

How do I submit my COVID-19 exemption request form once it has been completed? 

Prior to Submission

Before submitting your exemption request form through any means please verify the following prior to submission: 

  • All information fields on the form are filled in 
  • All initial boxes are reviewed and initialed 
  • All signatures, dates, and other information fields on the form are filled in 
  • Any required additional documentation is included 

Online digital copies (preferred method) 

You may submit digital copies of your completed exemption request form and any associated documentation via our Microsoft Forms "Submit a Vaccination Exemption Request” form

  • For question 1: Select the name of the form that you are submitting 
  • For question 2: Click the “Upload file” button, browse to where you have your files saved on your device, and then select the files you wish to upload and then wait until the upload of each file has completed. 
  • There is a checkbox at the bottom of the form titled “Send me an email receipt of my responses”. If you wish to have a receipt of your submission this is the only process to do so for digital file requests. You will not receive a confirmation email from the exemption form review team. 
  • Click the “Submit” button when you are ready to submit your request

On the Kent campus 

You may submit your completed printed exemption request form and any associated documentation at the “COVID-19 Vaccination Records and Exemption Requests Drop Box” outside of the main entrance of the DeWeese Health Center (1500 Eastway Dr Kent, OH 44242). 

On a regional campus 

Printed copies of each request form may be returned at the Student Services reception desk at each regional campus. The forms will then be sent to the review group at the Kent campus via on-campus mail.

What is the status of my exemption request? 

We do not currently have a dashboard or other means to check your request status. 

  • If you have submitted your exemption request online, then you can assume that your status is “Received”. You will receive an approved or denied message, in writing, once your request has been reviewed. 
  • If you have submitted your exemption request through one of the methods above for a printed copy, you will receive a “Received” message, in writing, once your request has been cataloged in our system. You will receive an approved or denied message, in writing, once your request has been reviewed.

The COVID-19 vaccination exemption processing and review team is not affiliated with the DeWeese Health Center or University Health Services.
