Kent Campus Immunization Requirements
Kent State University (Kent Campus) requires students to provide documentation of immunity to measles, mumps, and rubella (MMR).
- Acceptable documentation includes two MMR doses (after one year of age and separated by at least one month) or a positive antibody titer.
- Documentation is submitted through the DeWeese Health Center Patient Portal, including entering dates and uploading records.
- If you have not submitted your vaccine records to DeWeese Health Center, please take the following steps to submit your immunization records as soon as possible:
- Visit the DeWeese Health Center Patient Portal.
- Type in your Kent State University email and password. Sign in.
- Once you sign in, click on “Immunizations” on the left side of the screen.
- Follow the prompts at the top of the screen.
- Select each of the required vaccinations and enter the dates they were received. If you received a titer test, please enter the result date of the positive antibody titer test.
- Once all dates are entered, click submit.
- Click “Uploads” on the left side of the screen.
- Upload your documentation for your vaccinations. DeWeese will accept a copy of your original shot records, school records, or the Vaccination History Form found online.
- Submissions may take up to 30 business days to review and verify.
- If you do not have records, a licensed healthcare professional can complete the provided vaccination history form.

- DeWeese Health Center can help students with questions about requirements, documentation, care and by providing the MMR vaccine on site.
- Additionally, local health departments and pharmacies may offer MMR vaccination.
