1. Review your schedule for attendance expectations
Will you attend online, remote, or in person? If you are attending in person, proceed through this checklist before you enter campus.

2. Have you signed the KSU Pledge?
If YES, Proceed to item 3. If NO, do not come to campus.

3. Do you practice the Flashes Safe Seven Principles?
If YES, Proceed to item 4. If NO, do not come to campus.

4. Is your cloth face covering ready?
Have you received your facial covering or applied for accommodation? If YES, Proceed to item 5. If NO, do not come to campus.

5. Conduct daily self-check
Answer YES or NO to each question, if you answer yes to one or more questions immediately start self-isolation and remote instruction. Immediately call the Covid-Care Line (information placeholder) to discuss how the University will guide you in navigating testing, tracking, and returning to campus.
1. Do you have any symptoms as defined by the CDC?
- Fever or chills
- Cough
- Shortness of breath or difficulty breathing
- New loss of taste or smell
- Fatigue
- Muscle or body aches
- Headache
- Sore throat, congestion, or runny nose
- Nausea, vomiting, or diarrhea
2. Within the last 14 days, have you been in close contact with someone who has been diagnosed with COVID-19 or has been placed in isolation for COVID-19?
3. Have you had a COVID19 test prescribed by a medical professional because you have recently had symptoms and you are currently waiting for the results?
4. Have you been asked to self-isolate or quarantine by a medical professional or a local public health official?
