“Our team has found PDCM extremely helpful to ensure the student, parent, school, OOD, and DD are seamlessly working together to get the best outcomes for students. We have been able to hold our team accountable through scheduling monthly meetings, keeping a running agenda with speaking points to make sure our meetings are efficient, developing written procedures so everyone is on the same page when referring to outside agencies, and completing the backwards planning tool for our identified students. “As a result of our involvement with the PCDM project the JTC and Intervention specialist in our district have been able to dive deeply into increasing interagency interaction to improve transitions for students from school to adult living and careers. We also have been given support and tools to assist families. By participating in guided discussion and brainstorming, the team has been able to better articulate our needs to create a more successful transition program. The PCDM project is a great fit because no matter where a school is at in their creation of a transition program, the leaders meet the team where they are and encourage growth.” (Cohort 1 Team)
