Please complete this brief, 7-question survey about remote teaching. The first four questions are about you (name, remote teaching). The form also contains two questions asking if you would like to nominate yourself or a colleague for recognition for excellence in remote teaching during the Fall 2020 semester. Finally, the form asks for issues you have faced with the creation of remote content and the practice of remote instruction; we intend to review these responses to identify common challenges and attempt to address what we can.
