"Know Your Numbers" - Final On-site Health Screenings for 2017

As part of the Wellness Your Way incentive program, benefits-eligible employees are invited to attend an on-site health screening, or have one completed with their primary-care physician, by June 30. By completing a health screening (on-site or with your physician) and the online health risk assessment, employees are eligible to receive $150 added to their pay as a Tier One reward.

Completing Tier One Activities
If the health screening is completed on-site with Be Well Solutions, Kent State University's wellness partner, it automatically will be loaded into the employee’s personal portal. If the health screening is completed in a physician's office, the employee must login to the Be Well portal, under "My Wellness," select "Wellness Reward Program," print the "Form 1-Physician Screening Form" and take it to the appointment. Your completed form then should be submitted to Be Well Solutions using the address on the form. 

The following on-site dates are available prior to the June 30 deadline.

Please note that all Tier One requirements MUST be completed by June 30. Employees may work on their Tier Two activities simultaneously with Tier One, but they will not be eligible for their Tier Two $150 incentive until Tier One requirements are complete. Registration is required.

How to Schedule an Appointment 
Employees should login to their private Be Well portal at portal.bewelldata.com. Those who have never logged in previously will find instructions here for first-time access .

  • Once logged in, select "My Appointments," then "Worksite Screening." Choose the date and location. Employees should keep in mind that they will need to fast for 8 to 10 hours prior to the health screening and blood draw via a finger prick.
  • To complete the online health risk assessment while logged into Be Well, select "My Wellness" and then select "Health Risk Assessment."
  • Screening results remain confidential at all times.
