A structured planning tool that helps transition professionals work backwards from a student’s employment goal. By identifying the final objective and deadline, then mapping smaller, actionable steps with intermediate timelines, this guide breaks the process into manageable stages. It provides a clear roadmap that keeps the team focused, organized, and aligned with the student’s ultimate outcome.

Service Plan Alignment Form

A practical tool that helps teams monitor alignment across the IEP, IPE, and ISP. By sharing copies of each agency’s planning documents, teams can check for consistency, identify overlapping services, and ensure that supports are coordinated rather than duplicated. This form fosters collaboration among schools, VR, and DD services so all team members are working toward common goals for the student’s transition success.
