Student and other request forms can be accessed below using defined links. 

Dynamic Forms and Adobe Acrobat Sign are the new electronic signature tools for Kent State University.  We will be converting documents to Dynamic Forms by June 2023, replacing the current electronic signature tool, DocuSign.

There may be 2 links available to complete a form. Choose which form to complete based on your current situation.  Those that do not have an active @kent.edu email address will need to create an account.

  1. Current students (those with an active @kent.edu email address/FlashLine credentials)
  2. Non-current students (those that do not have an active @kent.edu email address/FlashLine credentials)

If you are a non-current student, an account will need to be created using a personal email with Dynamic Forms when completing the form.

Please have any required documentation ready to upload when completing your forms. You can view your draft/pending/completed Dynamic Forms after you have submitted your portion of the form at dynamicforms.kent.edu.

Some forms are not electronic and are completed manually (PDFs).  Each form is identified next to the link. 

  • Dynamic Forms or DocuSign: Submitted through the direct link and DO NOT need to be submitted manually. 
  • PDF forms: MUST be submitted manually as stated on the form.

Transcript Requests

Electronic exhange to Ohio public institutions (FlashLine)

  • Current and recently enrolled students log in to FlashLine and go to Student / Resources / Requests and Authorizations. Within this section, click on Request Official Transcript. On the Official Transcript Menu, click on 'Electronic Exchange to Ohio Public Institutions.

Electronic Transcripts 

  • Current and recently enrolled students log in to FlashLine and click on Request Official Transcript in the Student Category / Resources Page / Grades and Transcripts Section. On the Official Transcript Menu, click on 'eTranscript.'
  • Former students who cannot access FlashLine will need to create an account with Parchment.

  • If you are a 3rd party ordering on behalf of a student, you have the option of submitting a request via Parchment for an electronic transcript or paper transcript.

Student Legal Name Change Request (Dynamic Forms)

*Documentation is needed for name changes (except if changing middle name to middle initial). A Driver's License is not an acceptable form of documentation. Examples of acceptable certified legal documents include:

  1. Marriage – Marriage Certificate, Court Order, Social Security card or valid U.S. Passport
  2. Divorce – Court Order, valid U.S. Passport, Social Security card or Divorce Decree (must state name is restored to former name)
  3. Legal name change – Court Order, Adoption Order, Social Security card or valid U.S. Passport
  4. First name change – Birth Certificate, Social Security card or valid U.S. Passport

Chosen Name (FlashLine)

  • Students may submit a chosen name through FlashLine in their Student Dashboard by clicking on their profile picture, under the Secure Account Access section.

Change of Address, Phone Number, Email addresses, Emergency Contacts (FlashLine)

  • Students can update their addresses, phone numbers, email addresses and emergency contacts through FlashLine in their Student Dashboard by clicking on their profile picture, under the Secure Account Access section.

Bachelor's Students Awarded Associate Degree

Automatic Awarding of Associate Degree Opt-Out Form (Dynamic Form)

Those who have met the criteria will receive an email to their Kent State University email address letting them know of their eligibility for the associate degree. Regional Campus students with questions about the program should contact their academic advisors or their campus representative.

To qualify: 

  1. The student must be currently enrolled
  2. Have completed or be currently enrolled in courses that meet the requirements of an associate of arts or associate of science degree
  3. Have not previously been awarded a degree

Credit When It's Due

Credit When It's Due Opt-Out Form (Dynamic Form)

Students who have met the criteria will receive an email to their kent.edu address letting them know of their eligibility for the associates degree.  This opt-out form is only completed if you do not want an associate degree.

This form has has expired for Spring 2023.  It will be available in late October for Fall 2023.

Student Data Requests

The Office of the University Registrar takes the request for the release of a student’s information, whether student directory or non-student directory, very seriously. The Office of the University Registrar will ensure that all provisions of University Policy #5 - 08.101 are followed, as well as, require that requestors understand the implications of non-compliance to securing all student information. For additional information regarding third party data requests please visit the Public Records Requests on the Office of the General Counsel's website.
