Our Application Checklist

• Complete application for admission
• Submit official high school transcript
  • Sent directly from high school with official signature
• Official Individualized Education Plan (IEP) or 504 Plan
• Most recent official Evaluation Team Report (ETR)
  • (Conducted within the last three years) or a Psychological Evaluation including an IQ score
• Two reference forms
  • Not to be completed by family, we recommend one academic and one employment reference
• $100 application fee
