Certified Mail Receipt

When using a Certified Mail Receipt, the sender is able to track mail pieces. The receipt portion of the form should be completed by the sender. The U.S. Postal Service will round date/stamp the Certified Mail Receipt upon accepting the mail piece.

Please include your department name on the receipt. The receipt portion will be returned to the sender indicating applicable postage and fees. The bar coded portion will remain intact with the mail piece.
