Delivery Confirmation

Delivery Confirmation allows the sender to track mail pieces within the United States only. The U.S. Postal Service will round date/stamp the Delivery Confirmation Receipt upon accepting the mail piece. The receipt portion will be returned to the sender. The bar coded portion will remain intact with the mail piece.

The Receipt portion of the form should be completed by the sender, including the department name. The receipt is returned to the department indicating applicable postage and fees.
