Change in Program of Study
This form may only be used by students currently enrolled in, or on an approved leave of absence from, a graduate program of study at Kent State University. Students must contact the Graduate Coordinator of the proposed program to determine whether they should submit 1) a Change in Program of Study form or 2) a new application. International students must consult with the Office of Global Education to determine how a change of program might affect their visa status.
This form MAY be used to:
- change to a different degree program, major or concentration
- add a concentration
- change from a degree program to a certificate program
- change catalog year while maintaining your current degree program
- withdraw from one part of a dual or multiple program of study (dual degree, multiple major, etc.)
This form CANNOT be used to:
- add a degree program, major or certificate
- change from a certificate to a degree program
- withdraw entirely from a program and the university

Combined Bachelor's / Master's Degree Program
A combined bachelor's/master's degree program has established admissions requirements, a curriculum and graduation requirements. To qualify for a combined bachelor's/master's degree program, an undergraduate student must complete a standard online graduate application (including the application fee) and be admitted to the graduate program of interest.
The Combined Bachelor's/Master's Degree Program form must be submitted prior to the first semester of enrollment in the combined program.

Graduate Assistantship Application (PDF)
Graduate appointments are designed both to serve the needs of the University and to assist in the professional development of the student. A graduate assistant assists with instruction, research, or administrative duties beneficial to the appointee's professional development and to the employing department or office. Master's, educational specialists, and doctoral students are eligible.
A graduate assistantship is an appointment made by Kent State University to full-time graduates who display a high level of scholarship and academic excellence. The award is merit-based, not need-based, and includes stipend and tuition.

Request for Exception to Full-Time Enrollment for Graduate Assistants
This form is to be used for Graduate Assistants nearing completion of their coursework requirements, but needing fewer than eight (8) credit hours to complete their degree work. Students must be registered for at least one (1) credit hour. Other extenuating circumstances may be considered but typically will not be granted. This form must be initiated by the student, reviewed and signed by the employing department's Chair/Director and the appropriate College Dean before submission to the Graduate College. Because this request is for exception to a university enrollment requirement, final approval is at the discretion of the Dean of the Graduate College.

Leave of Absence / Re-Enrollment
Leave of Absence
A Leave of Absence is granted for degree-seeking graduate students in active status who must be away from their studies for personal, family, financial or other compelling reasons. Access this form through the link below or through FlashLine, within Student / Resources / Requests and Authorizations.
Graduate Re-Enrollment
This form is only for students returning from an approved leave of absence. Students not on an approved leave of absence, who withdrew from or left the University one ore more years ago, must formally apply and follow the same admission procedures required of new applicants. See policy #3-01.13.

Request for Overload
University policy allows students to register for a maximum of 6 hours in Summer 1 and 3, 10 hours in Summer 2, 10 hours overlapping Summer 1 & 2 or Summer 2 & 3, 12 hours for all summer sessions combined. The maximum number of hours for Fall or Spring is 16.
In order to have an overload considered for approval, you must present a substantive written rationale. Be sure to address reasons why you are unable to enroll for the class at another time.

Request for Transfer of Graduate Credit
Graduate courses earned at another college or university may be transferred into a Kent State University graduate program. Students who wish to have those courses considered for transfer must petition administrators of the graduate program. The program can accept or reject such courses based on the program requirements, regardless of the discipline or institution in which the credits were earned. Official transcripts MUST accompany this form.

Graduate-level teacher candidates meet all program requirements to be fully prepared for student teaching:
- 3.0 GPA
- All courses completed
- Approval of their faculty advisors

- Dissertation director/advisor representing the student's program area.
- Dissertation committee member or Co-Advisor from the student's program area or school.
- Outside member — member from a discipline outside the student's program area.
- Optional member(s) may be added at the discretion of student and director.

Readiness to Defend Dissertation and Submission of Brochure
Committee members are to sign this form after the Pre-Defense meeting. Students are to provide defense brochures for committee members and additional copies for guests attending the defense. The Office of Graduate Student Services will post a schedule of upcoming defenses.
