Revise the Name of a Major

Complete the following:

1. CIM Program Management proposal
   • For help, consult the Curriculum Management User Manual and frequently asked questions (FAQ)
   • Attach any supporting document
     • Formal letter, memorandum of understanding or other type of correspondence, if applicable, that outlines the agreement to deliver the program at a site
2. If you are also making curricular changes, please establish new courses and/or revise existing courses through CIM Course Management
3. Curricular Request Form to obtain the appropriate external paperwork needed for a name change
