Daily Log
OFF-SITE LOCATION AND TRIP REPORTING FORM
For annual Clery reporting purposes, the Kent State University must collect crime statistics for those areas the university owns or has control over. Department representatives must complete this form for any university-sponsored trips, programs or activities conducted at an off-campus site that include students, regardless of whether an incident occurred at the site. Student teaching, medical practicums and College Credit Plus (CCP) programs do NOT need to be reported unless an agreement exists between Kent State and the site for a specific space at the site to be controlled by Kent State.

The Daily Crime Log
The purpose of the daily crime log is to record criminal incidents and alleged criminal incidents that are reported to the campus police. This crime log represents the most recent 60-day period. A list of crimes that is older than 60 days is available within two business days upon request.
The Fire Log
The purpose of the fire log is to record actual fires that occur in on campus student housing facilities either accidental, intentional or by unknown cause. This log represents the most recent 60-day period. A list of fires that is older than 60 days is available within two business days upon request.
