Departments may use this form to request payment from their foundation accounts, and individuals who need reimbursed for expenses that are not paid for by the university, such as donor entertainment expense, may also use this form. The funding request must be for the purpose stated in the completed documentation from the fund creation (MOU, GIF, Spendable Fund Authorization Form). The mission of the Kent State University Foundation is to support Kent State University, and as such, most of the payments made out of the Foundation are grants to the university. The majority of these payments are for scholarships awarded, lecture series and various events that support the departments. The department initially pays the funds out of their university index, then to redistribute the funds back into their index, departments must submit this funding request (being sure to include the fund they want the funds to come from and the university index they need the funds to go to). Once received, the funding request goes through an approval process to ensure the funds being requested are being used in accordance with the Foundation Fund documentation.
