Student Forms
The Honors College is now using Dynamic Forms to digitally submit, route, and complete the majority of student forms.
- You will be prompted to sign in with your FlashLine credentials to view and/or submit these forms. Complete your portion of the form (paying special attention to submitting accurate email addresses wherever prompted,) and click the submit button.
- Forms will route through each level of reviewers/approvers and collect their digital signatures. Reviewers/approvers will receive an email notifying them of the pending form. Students receive notifications at every step of this process.
- Please note, forms with (PDF) in the title cannot be completed via Dynamic Forms. Please download and complete the Internship Contract, Research Experience Contract, and Senior Honors Thesis Oral Defense Forms. These may be submitted in person to the Honors College or scanned and emailed.
The following academic forms are utilized to pursue various Honors College course and programming options. Forms must be completed in their entirety and submitted prior to the deadlines disclosed.
Class Project Agreement Form
The deadline to submit for Fall 2024 is Friday, August 23 at noon.
The Class Project Agreement form is submitted to create an Honors section of a standard KSU non-Honors course with the approval of the course instructor.
Honors College Student Agreement
(This will be completed with your assigned Honors advisor during your first semester advising appointment)
With the completion of the Honors College Student Member Agreement Form the student has agreed to fulfill all requirements necessary for continued participation and scholarship eligibility in the Kent Sate University Honors College.
Individual Honors Work Form
The deadline to submit for Fall 2024 is Wednesday, August 21 at noon.
The Individual Honors Work form is submitted to pursue independent study as a form of Honors credit.
Internship Contract (PDF)
The deadline to submit is noon on the last day of the semester in which you completed the internship.
The Internship Contract is submitted for students to receive 3 internal Honors hours for completing an internship of at least 150 hours.
Research Experience Contract (PDF)
TThe deadline to submit is noon on the last day of the semester in which you completed the research experience.
The Research Experience Contract is submitted for students to receive 3 internal Honors hours for completing a research experience of at least 150 hours.
Senior Honors Portfolio Form
The deadline to submit for Fall 2024 is Wednesday, August 21 at noon.
The Senior Honors Portfolio form is submitted for students seeking one Honors credit hour through the completion of a reflective portfolio.
Senior Honors Thesis Proposal Preparation Form
The deadline to submit for Fall 2024 is Wednesday, August 21 at noon.
The Senior Honors Thesis Proposal Preparation form is submitted for students seeking to register for the 1 or 2 credit-hour thesis proposal preparation during the semester before their senior year.
Senior Honors Thesis/Project Proposal Form
The deadline to submit for Fall 2024 is Wednesday, August 21 at noon.
The Senior Honors Thesis/Project Proposal form is submitted after the successful completion of the preparation semester. The thesis proposal is a formal document that sets forth the parameters of the intended work.
Senior Honors Thesis Defense Committee
Form declares participating members of the thesis defense committee.
Senior Honors Thesis Oral Defense Form (PDF)
Form is completed and submitted immediately following the oral defense.
