Nursing Alumni Award Nomination
As part of our 50th Anniversary celebration, we are launching our Distinguished Alumni Award. Nominate an outstanding Kent State Ashtabula nursing graduate for consideration using this form, or access a PDF version of this form below.
Criteria for selecting the 2016 Distinguished Nursing Alumni Award winner are as follows:
- Nominee is a graduate of the Kent State Ashtabula Associate Degree in Nursing program
- Nominee demonstrates distinguished service to the field of nursing
- Nominee has made significant contributions to the profession of nursing
Please consider these criteria when writing your nomination and provide examples of how your nominee has met each. Your thoughtful and descriptive responses in conveying why your nominee is worthy of recognition will affect your nominee's chances of being recognized.
Nominations will be accepted from August 1 through September 20, 2016 for this year's award. The award winner will be recognized at the "Celebrating 50 Years of Excellence in Nursing Education" event on Saturday, October 1, 2016.
