Employee Financial Policy

Payment Terms

Payment is expected at the time of service.  DeWeese Health Center accepts cash, checks, or VISA/Mastercard/Discover.  All unpaid charges will be billed to the employee.


A. Insured KSU Employees

Insured KSU employees are eligible employees who have health insurance coverage.

DeWeese Health Center is a primary care Preferred Provider Organization for insurance carriers that contract with DeWeese.  The list includes KSU's Medical Mutual employee plans, along with Aetna, Anthem BC/BS, CIGNA, Medical Mutual of Ohio, UnitedHealthcare and many others.

DeWeese Health Center will submit claims to all insurance companies (with the exception of Medicare), regardless of our network participation.

1. Office Visits

Office Visits refer to evaluation and management services provided to employees in our medical clinics.

COPAYS:  Copays are due at the time of service and the Primary Care Provider (PCP) copay applies.

2. Laboratory Services

DeWeese Health Center provides onsite laboratory services for basic tests such as urine analysis, strep, mono and HIV.  Payment made by the insurance company for laboratory services is accepted as payment in full.  You may incur additional charges if you have a deductible or if the insurance company does not cover the services billed.

DeWeese Health Center contracts with Quest Diagnostics as a reference lab for all tests that cannot be performed at DeWeese. Quest will bill your insurance directly and you may receive a statement from Quest for the patient responsibility portion of your laboratory charges.

3. Procedures, Services, and Supplies

Procedures and Services include services rendered by DeWeese Health Center's Radiology department.

You may have out of pocket expenses if you have a deductible, or co-insurance, or if the insurance company does not cover the services billed.

4. Pharmacy

DeWeese Health Center Pharmacy participates in the Caremark Prescription plan network.  your Caremark copay applies and is payable at the time of service at the checkout desk.

DeWeese Health Center will fill all prescriptions, but can only submit prescription claims to Caremark.

DeWeese Health Center  carries many over-the-counter (OTC) supplies and items at below retail prices.


B. Self-Pay KSU Employees

Self-Pay KSU employees are eligible employees who do not have health insurance.

1. Office Visits

For eligible employees, there is a minimal fee for office visits provided to KSU employees in our medical clinics.  Payment is due at the time of service.

2. Laboratory Services

There are reduced fees for laboratory services provided onsite by DeWeese Health Center.

DeWeese Health Center contracts with Quest Diagnostics as a reference lab for all tests that cannot be performed at DeWeese.  Payment is due to DeWeese Health Center at the time of service.

Procedures and Services include services rendered by DeWeese Health Center's Radiology department.

For eligible employees, there is a reduced employee self-pay fee schedule for procedures and services.

4. Pharmacy
